to download the Traffic Light Form.  This is for people who may be admitted to hospital or go into hospital as an outpatient.  This form is for parents or carers to complete.

This assessment gives hospital staff important information about you. 

Please take it with you if you have to go into hospital.

Ask the hospital staff to hang it on the end of your bed and make sure that all the nurses who look after you whilst in hospital read this assessment.

Racial and Hate Crime Incident Report Form

This is the form for reporting racial and hate crime incidents if something has happened to you that made you feel upset, frightened and/or scared. 

Please make sure you talk to your family, social worker, key worker or day centre staff and they can talk to you about what happened, getting you support if you need it and can help you to fill out this form to report what happened.
